The corporate circle of confusion.
Some people make a living off of perpetuating it. |

There is so much truth in that.
With doctors and lawyers, "errors" means "malpractice" so the stakes are very high. In other industries, I think errors leads to losing customers, which is obviously not sustainable in the long run, but not the instantaneous game over that it is for doctors and lawyers. |

The next step is to add direct connecting lines between the opposing stages; so, the intermediate stages in the circle can be bypassed to create internal conflict more efficiently.
I saw that being done by US Army Training and Doctrine (TRADOC) Command back the in the 1980s. ![]() |

hiring well, training relentlessly, and holding people accountable break that cycle.
most companies are too cheap to invest in the training, and too lazy to fire slackers. not to mention clueless on how to hire. that's my opinion.

"From this day forward, Flight Control will be known by two words: ‘Tough’ and ‘Competent.’ Tough means we are forever accountable for what we do or what we fail to do. We will never again compromise our responsibilities. Every time we walk into Mission Control we will know what we stand for. Competent means we will never take anything for granted. We will never be found short in our knowledge and in our skills. Mission Control will be perfect. When you leave this meeting today you will go to your office and the first thing you will do there is to write ‘Tough and Competent’ on your blackboards. It will never be erased. Each day when you enter the room these words will remind you of the price paid by Grissom, White, and Chaffee. These words are the price of admission to the ranks of Mission Control.” -Gene Kranz I read this today in Whitehouses' book, Apollo 11. It's an interesting read which also draws back the shades on how similar issues were responded to by the Russians.

On the top edge of my monitor there is a strip of paper taped which reads, 'it is unacceptable to do less than you are capable of doing'
I hate it when people are lazy. That tells me that I dislike it when I am. So I watch it.

I cannot believe how much effort some people put into being lazy. My cow-orker and I often marvel at the extreme length people will go to to avoid walking 20 yards to get the right piece of equipment for the job. No amount of training seems to fix stupidity
