More precisely, it's usually done in a room with as many relatives as can fit. In most cases there is a small family gathering, like a baby shower, and when the time comes the baby, moyel, parents, grandparents, godparents, and anyone else who wants to go into a side room to witness the bris.
There are all sorts of sects and traditions within Judaism, but I have never heard of anyone using their mouth before.
OnyxCougar posted a link to Jewfaq, which I like as a resource.
