Whenever there's a huge error made, my VP talks about how we're going to do everything with written procedures and training, but does he enforcement it among his managers? No. So when everyone on the team does the same thing in different ways, that's on him.

All he would have to do is ask his people, "Hey what did you do about procedures? Show me the procedures your team uses."
