A tapeworm will do the trick. Those suckers can grow to incredible lengths - all wrapped up in your intestines, but they don't add hardly anything to your weight or cause an extended belly. They just siphon off your nutrition and grow longer and longer... I read somewhere that one person had a tapeworm so long that it extended into his mouth. That was probably a myth, but the rest isn't.

Back around the turn of the last century, hucksters sold bits of live tapeworm as a weight reduction aid. They worked, but the side effects were rather unpleasant.
