true, and that is one reason public schools have become such a joke in some areas. teachers with tenure just waiting for their pension while not giving a damn about the education they are being paid to provide. No Child Left Behind is layer upon layer of misguided BS, surrounding a kernel of a good idea - make educators accountable for the progress of their students. good idea + government = poor execution, but the idea has merit. but that is a different thread...

nurses work hard for a fixed wage

nurses frequently move from organization to organization for better pay, better working conditions, greater room for advancement. hardly a "fixed wage" situation.

why would doctors be any different?

while some doctors have altruistic motivations, i would expect that they subjected themselves to the difficult schooling and cutthroat residencies while racking up massive debt for the purpose of making $$$. they specialize to make more $$$. they own their own practices so they can keep more of the $$$. while there are many exceptions, the most skilled and brightest among us are typically motivated by A) money, B) Power, C) recognition. whether those are worthy goals or not, is another discussion, but you can't deny that it is so.

If you are attempting to change a universal healthcare system, paid for by general taxation and national insurance

but see - i'm not. i think universal healthcare paid for by taxes is a bunch of crap. competition creates innovation, innovation leads to progress. do some get left by the wayside? yep. that sucks but it is life. i don't agree with lowering the upside of anything just so that the people at the top and the bottom can be closer to each other. lumping everyone into any single system limits any motivation for improving things. if that system is part of a government run organization makes it damn near impossible to ever actually achieve the goal you set out with - providing top notch healthcare for everyone.
