"Deserve" is another way to say "earned". You deserve something if you've earned it. They've also earned their paychecks, so they deserve them. Notice I haven't said anything about the actual quality of their wages.

Mercenary earned his government-run health care thru service - good or bad, he deserves to receive it. I don't see anything funny, or complicated, about it...
