It's for joining sheets of paper together without using staples. We had something similar at school (I really am getting old). When you hit the knob it'll cut a hole like a normal hole-punch only longer and straight, next to that it cuts a little tab which you then fold up inside the hole to hold a few sheets together.

My wife bought one of those and more than once she has sent me to work to scan documents that have had key things like dates or other information obliterated by this contraption. Good thing is that if you are really careful you can unfold it and put it back together to scan.
